Product Description
Syncet tablet is the combination of levocetirizine, phenylephrine, paracetamol and ambroxol. This combination of drug comes under cough and cold preparations which are safe and effective for patients. This combination of drug can be taken with food if stomach upset occurs. In this combination paracetamol provides analgesic, antipyretic and nonnarcotic agent, ambroxol is a mucolytic agent, levocetirizine produces action by blocking H1 histamine receptors and phenylephrine is a direct acting sympathomimetic agent.

Mechanism of action
Ambroxol is a metabolite of bromhexine and is a mucolytic agent.
Paracetamol causes inhibition of COX and a selective COX 2 inhibitor. It has analgesic and anti-pyretic properties.
Phenylephrine is a powerful vasoconstrictor and produces local and systematic action on by acting on α1-adrenergic receptors peripheral vascular smooth muscle.
Levocetirizine
It competes with histamine for binding at H1 receptor on the effectors cell surface which causes the suppression histaminic edema and pruritus. The low incidence of sedation is because the reduced penetration of cetirizine into CNS.

It is well absorbed in the body with mean peak plasma concentration (Cmax) of 114 ng/mL at a time (Tmax) of 2.2 hours
Distribution
It is well distributed in the body with 93% plasma protein binding. Vd is 0.41kg
Excretion
The plasma half life is 7.9 ± 1.9 hours; total body clearance is 0.63 ml/min/kg
PHENYLEPHRINE
Absorption
It is absorbed after oral administration with reduced bioavailability about 38%. It follows first pass metabolism with peak serum concentration of 0.75-2 hours.
Distribution
The protein binding of the drug is about 95%
Excretion
Phenylephrine and its metabolites are excreted in urine
AMBROXOL
Absorption
The absorption of drug is rapid and complete
Distribution
The protein binding of the drug is through urine
Excretion
The half life of the drug is about 7-12hrs.
Paracetamol

It has rapid and complete absorption
Distribution
The protein binding of the drug is about 25%, it undergoes glucuronidation in liver.
Excretion
About 80% of acetaminophen is excreted in the urine after conjugation and about 3% of the drug is excreted unchanged.
Pharmacology
Contraindications
- Patients with hypersensitivity to such drugs
- Renal disease
- Pediatric patients with renal impairment
- Hypersensitivity to any of the product

Precautions
- Long treatment can cause somnolence, fatigue, and asthenia
- Patient should not involved in those task which require complete concentration
- Patients with predisposing factors of urinary retention
